Application by downloading the file and mailing the
completed form with the application fee to the address referred to below.
To download an of the documents listed on this page, simply right-click on the
link to that document and select "save target as..." You may then choose
a place on you computer to store and print the document.

Secure the Application for Admission form which is included in the catalog or download a printable version. Attach a recent photograph to the completed form and send it with the $20.00 application fee to the:

Complete the
first section of the Pastor's Recommendation,
then give it to your pastor.
Complete the first section of the two General Recommendation forms, then give them to two adults who know you quite well. This form should not be given to relatives.
Complete a Transcript Request, and mail it to the high school from which you graduated. If you have not yet graduated, request the high school to send a transcript of your course work completed up until the current semester. If you passed the Tests of General Educational Development (GED), you must have an official copy sent directly from the GED test center to the Director of Admissions. If you expect to transfer credits from another college to Commonwealth Baptist College, mail a completed Transcript Request to your former college.
Request official copies of the ACT scores to be sent to the Director of Admissions.
Request work scholarship applications if needed.
Complete the medical/health form which will be sent to you or you may download.
